My kitchen faucet and sprayer leak at the faucet/sink level - do I need to replace the faucet?

There is no leakage under the sink. Water leaks from under the faucet plate and runs out onto the counter top. Is the faucet poorly installed? Do I need to replace the entire faucet or maybe just a gasket?
